UNA DOLOROSA INTROMISSIONE NELLA SESSUALITÀ DELLA ADOLESCENTE

RIASSUNTO
L’Autore cerca di dare una differente interpretazione del fenomeno della gravidanza in adolescenza. Egli non si limita a considerarlo un incidente di percorso, ma iscrive l’evento nell’ambito del processo di sviluppo dell’adolescente stessa. Vengono proposti e sviluppati tre livelli di interpretazione: la gravidanza come una verifica della funzionalità dei propri organi genitali, il desiderio di un bambino come «oggetto» che ripara alle carenze affettive della giovane madre e, infine, la gravidanza come una condotta a rischio, nel tentativo di colpire il corpo sessuato.

SUMMARY
Pregnancy: a painful interference in adolescent sexuality. When pregnancy occurs in adolescence, it is most often attributed to an «accident», and the medical-social response is to provide better information about contraception. But when we look closer, we often see a logic other than that of an accident: adolescent girls who express, when they are permit- ted to, a desire for pregnancy or for a child. Why is an adolescent girl sometimes invaded by this desire, to the point of not being able to put it off even for a short time, and why is she ready to sacrifice a part of her adolescence to it? The Author describes three possible answers to the question: pregnancy is a means to test one’s fertility; it helps the young mother cope with her emotional deprivation; it is the consequence of a risky behavior against the sexual body.

CONCEPIRE E ABORTIRE TRA FANTASIA E REALTÀ

RIASSUNTO
L’Autrice analizza le varie e contraddittorie valenze relative al concepimento e all’aborto in adolescenza, sia fantasticati che realizzati, e gli effetti di interferenza a più livelli sul processo adolescenziale. In particolare, quando tali evenienze si verificano nel corso di una psicoterapia psicoanalitica si delinea un significato correlato a uno specifico passaggio cruciale: quando le condizioni di dipendenza nella regressione si annunciano come stabili e affidabili si verifica il rimanere incinta con l’unico sbocco nell’aborto. La gravidanza si configura come incidente che condensa vari significati di passaggio all’atto, occasione potenziale di questionare le proprie origini e il senso del proprio concepimento a posteriori, di riprendere contatto col vero Sé potenziale rappresentato dall’embrione. Tuttavia, avvicinandosi alle fonti del trauma, emerge il rischio che venga abortito con l’embrione anche il processo innescato di potenziale elaborazione, con attacco al legame verso il terapeuta e interruzione della psicoterapia.

SUMMARY
Conceiving and aborting between fantasy and reality. The Author analyses the various and contradictory attitudes shown towards adolescent girls’pregnancies and abortions and their influence, on different levels, on the adolescent process. In particular, when such incidents occur during a psychoanalytic psychotherapy they are to be connected with a crucial passage: when dependence due to regression shows steady and reliable, becoming pregnant inevitably brings forth an abortion. Pregnancy can be an experience in which the many possible reasons for an acting out are synthesized; it is an accident, an opportunity to investigate about one’s own conception, reestablishing a contact with the real Self the em- brio stands for. However, getting closer to the origin of trauma, implies the risk, for the working out process, to be aborted along with the embrio, through an attack on the relationship with the therapist and the interruption of the treatment.

UN CASO PARTICOLARE DI CONCEPIMENTO

RIASSUNTO
Questo scritto presenta le difficoltà, sul piano dell’assetto analitico, della riflessione controtransferale e della teoria della tecnica che si incontrano nel lavoro di riparazione ed elaborazione di un concepimento affrettato in adolescenza, per di più sfociato in un aborto vissuto come imposto. Con la particolarità che Antonella aveva già subito un trauma, a livello del processo di soggettualizzazione (Cahn 2009), per il fatto di essere stata adottata. Come la presentazione clinica intende testimoniare, è opportuno usare molta cautela nel proporre precocemente un setting con sedute frequenti sul divano ad adolescenti traumatizzati o con basi narcisistiche fragili o assenti. Può trattarsi di un scelta affrettata da enactment da parte dell’analista. In queste situazioni, l’orienta- mento che sembra prevalere sempre di più con gli adolescenti è di assumere inizialmente e per un tempo a volte indefinito la posizione faccia a faccia.

SUMMARY
A peculiar case of conception. This work aims at showing the difficulties reparation and working out following a hasty pregnancy in adolescence are likely to cause, especially when abortion is experienced as an imposition. Such difficulties may be evident in the setting as well as in the countertransference and even regard the theory of technique. In Antonella’s case, the peculiarity consisted in her having already experienced, in her subjectualization process (Cahn 2009), the trauma of being adopted, shortly after her birth, by a sterile couple. As the clinical presentation in- tends to show, extreme caution must be used in presenting an early set- ting, with frequent sofa sessions, to adolescents who have been traumatized or have poor or not-existent narcissistic foundations, to avoid an «enactment»on the psychoanalyst’s part. In such cases, the prevalent trend with adolescents is to privilege, initially and for an undefined time, a «vis a vis»setting.

UNA MOSTRUOSA INTIMITÀ

RIASSUNTO
L’Autrice, attraverso il racconto di un’esperienza clinica, si sofferma ad analizzare come la gravidanza e la maternità siano state «svuotate» del contenuto pulsionale e quindi desessualizzate. Più specificatamente, descrivendo la mostruosa intimitàtra madre e figlia, approfondisce la tematica del rifiuto silenzioso della femminilità e il mancato passaggio, generazionale, da una sessualità infantile, sexuale, a una adulta, sessuale.
L’Autrice, inoltre, lascia aperti degli interrogativi sul significato della gravidanza in adolescenza, ipotizzando principalmente come essa possa rappresentare un tentativo di risarcire, con il futuro figlio, la propria madre per il fatto di non essere potuta rimanere la bambina bisognosa che le aveva riempito la vita.

SUMMARY
A monstrous intimacy. Through her report on a clinical experience, the Author considers how pregnancy and motherhood have been deprived of their instinctual content and desexualized. Describing the «monstrous intimacy»between a mother and her daughter, she delves into the theme concerning the silent rejection of femininity and the failed generation passage from a childish («sexual») form of sexuality to an adult («sessuale») one.
Moreover, the Author leaves some questions about the meaning of pregnancy in adolescence unanswered, mainly conjecturing it to represent, for a girl, an attempt to use the child’s birth to compensate her mother for the loss of herself as the needy child who had filled out her life.

L’ANGUSTIA DEL GINECOLOGO

RIASSUNTO
L’Autrice attinge dalla sua esperienza ventennale di ginecologo e psicoanalista per tracciare un quadro di come sia andato trasformandosi il rapporto fra le adolescenti e l’evento gravidanza. Una gravidanza in adolescenza rappresenta sempre un evento traumatico per le scelte indifferibili che essa comporta sia per la ragazza che per il proprio entourage, negli anni, sempre più coinvolto. Per gli stessi operatori sanitari che intervengono a vario titolo dal momento dello svelamento della gravidanza, la scelta dell’adolescente rappresenta una sollecitazione impor- tante sul tema, di difficile soluzione, del «che cosa sarà meglio fare?».
L’articolo è corredato da un caso clinico di una sedicenne che l’Autrice ha incontrato presso il servizio dell’AIED.

SUMMARY
A gynecologyst’s worry. The Author relies on her 20 year experience as a gynecologist and a psychoanalyst to draw a picture of the change occurred in the adolescent girls’attitude to pregnancy. Pregnancy in adolescence is always traumatic for the unavoidable choice it requires on the girl’s part and on her own, increasingly involved, entourage. For health caretakers, intervening on the girl’s behalf through their different roles, the girl’s choice represents an important stimulus to consider «what the best choice may be».
The article also describes the treatment of a 16 year old the author met at the AIED service.

L’INSOSTENIBILE LEGGEREZZA DEL SENTIRE.
UNA FIGLIA DIVENTATA MADRE

RIASSUNTO
Attraverso il resoconto clinico di una psicoterapia con una adolescente incinta l’autrice descrive la gravidanza in adolescenza come l’espressione agita di un mondo emotivo molto conflittuale e difficilmente integrabile. Viene riconosciuta come finalità, più o meno inconscia, non un autentico desiderio di maternità bensì il bisogno di trovare una risposta al dilemma fusionale/separativo nella relazione con gli oggetti interni e in particolare con l’oggetto materno.

SUMMARY
The unbearable lightness of feeling a daughter has become a mother.
Through the clinical record of the treatment of a pregnant adolescent, the author describes pregnancy in adolescence as the acted expression of a very conflicting emotional world which can hardly become integrated. The need to find an answer to the dilemma concerning the fusion/separation from the inner objects, especially from the mother, rather than an authentic wish to have a child, is acknowledged as the main, more or less sub- conscious goal, under those circumstances.

PER AIUTARLI A CRESCERE

LA FIGURA DEL COMPAGNO ADULTO DI FRONTE ALLA GRAVIDANZA DELLA ADOLESCENTE

RIASSUNTO
In questo elaborato le autrici condividono le loro osservazioni rispetto alla personale esperienza di compagno adulto di adolescenti incinte. La riflessione riguarda la possibilità di rendersi fruibili durante la gravidanza nel tentativo di mantenere vive delle quote di investimento adolescenziali importanti per la ripresa evolutiva delle ragazze. Il lavoro è sostenuto da presentazioni esperienziali che evidenziano la specificità dell’intervento con le adolescenti in carico, soggetti impegnati in un doppio compito evolutivo: da una parte il rimaneggiamento della propria identità, dall’altra, nonostante le implicazioni di chiusura di un possibile percorso evolutivo, la possibilità di rilanciare un’apertura. Importante è il riferimento al lavoro di gruppo intra e inter istituzionale di cui gli interventi hanno beneficiato con la conseguente possibilità di dare continuità e rafforzare la presa in carico, fornendo e garantendo così l’efficacia dell’intervento.

SUMMARY
The adult companion’s figure and his facing an adolescent’s pregnancy. In this work the authors present their observations about their personal experience as adult companions of pregnant adolescents.
Their reflection regards the opportunity to help the girls keep some adolescent investment quotes alive that may help the recovery of their development. The work is backed by the presentation of some experiences which show the specificity of the intervention with the adolescents in care, engaged in a double developmental task: on one side, the handling of their own identity, on the other the possibility to elicit their natural development even if their condition implies its closure. The inter/intra institutional group work is shown as important in so far as it has accounted for the continuity and the reinforcement of caretaking and the efficacy of the intervention.

DIVENTARE MADRI PER ESSERE FIGLIE

RIASSUNTO
Il lavoro, attraverso l’analisi delle risposte di adolescenti a un’intervista semi-strutturata, illustra come la gravidanza sia un acting per colmare una profonda deprivazione affettiva materna; la gravidanza diventa un «mezzo» per essere riconosciute figlie diventando madri. Le Autrici descrivono una doppia identificazione delle adolescenti: con il bambino che portano in grembo e con la loro madre. Viene inoltre sottolineato che le giovani in questione, oltre ai compiti propri del momento evolutivo, si trovano a fare i conti con le dinamiche relative alla gravidanza. In questa situazione le adolescenti, concentrate su se stesse, non sono in grado di «vedere» il bambino in quanto prese dalla soddisfazione narcisistica. Secondo le Autrici si viene a creare un’impasse evolutiva che fa rimanere le ragazze «ancorate» alla fase preadolescenziale e «abortire» la propria adolescenza.

SUMMARY
Becoming mothers to remain daughters. In this work, the answers to a semi-structured interview show how pregnancy is an acting used to com- ply with a deep motherly love deprivation. As such, pregnancy becomes a way to become mothers in order to be recognized as daughters. The Authors describe the adolescents’double identification with the child they bear and with their mothers. They also highlight how the young mothers have to deal with the dynamics set forth by their pregnancies, while coping with the developmental task of their age. In that situation, the adolescents look over their own narcissistic satisfaction rather than over their child.
The Authors hold that the resulting developmental impasse makes the girls stagnate into the pre-adolescent stage and abort their adolescence.

ADOLESCENTI CHE PARLANO CON IL CORPO

RIASSUNTO
Il lavoro presenta, a partire da due situazioni cliniche, alcune riflessioni sulla possibilità di osservare e analizzare varie peculiari comunicazioni del corpo nello spazio e nel tempo della seduta. L’interrogativo che orienta il lavoro riguarda le potenzialità terapeutiche della sintonizzazione, dell’osservazione e del tentativo di comprensione di queste comunicazioni. Allenare una capacità di ascolto e di attenzione alla dimensione corporea può essere uno strumento utile soprattutto nella terapia con gli adolescenti in quanto può favorire e stimolare la naturale propensione a simbolizzare, agli albori in questa fase evolutiva.

SUMMARY
Adolescents speaking through their bodies. Moving from two clinical cases, the work presents some reflections about the opportunity to observe and analyse some peculiar types of body communication in the space and time of the analytic session. The work examines the therapeutic potentialities of tuning, observing and venturing to understand such forms of communication. Training one’s ability to pay attention to the body dimension, taking it into due account, may be of some utility, especially in the treatment of adolescents, since it may enhance their natural, though just arising, attitude to symbolize.
